Palliative Care – Comfort and Support at Home

At A Choice Home Health Care, our Palliative Care program provides specialized medical support for patients living with serious illnesses. The focus is not on curing the disease, but on relieving symptoms, reducing stress, preventing avoidable hospitalizations, and improving or maintaining quality of life—for both the patient and their family.

Purpose of Palliative Care

To help patients remain comfortable at home while continuing treatment for their illness.

To support families in understanding care needs and accessing available resources.

To step in early, whenever possible, to address problems before they lead to hospitalization.

Our Goals in Palliative Care

  • Symptom Relief: Managing pain, breathing difficulties, fatigue, nausea, and other discomforts.
  • Emotional & Spiritual Support: Helping patients and families cope with stress, anxiety, or depression.
  • Quality of Life: When possible, improving comfort and independence; when not possible, helping maintain stability and delaying deterioration.
  • Rehabilitation Support: Through therapies such as maintenance physical therapy, patients may preserve mobility, strength, and daily function longer.
  • Family Guidance: Teaching caregivers how to provide safe, effective support and connecting them with needed resources.
  • Care Coordination: Our team works closely with doctors, specialists, and caregivers to ensure care is seamless and consistent.

How It Differs From Hospice Care

Palliative care can begin at any stage of a serious illness and may be provided alongside ongoing treatment. It is not limited to end-of-life care (the focus of hospice).
